- The 13-digit Local Registration Number (LRN) on the baby's birth certificate OR the unique registration code from the letter you may have received.

- County where the student attended school (based on the chart below),
- Their birthdate,
- Their 10-digit Statewide Student Identifier (SSID) OR the unique registration code on the letter you may have received.
